I can't take this report seriously.
The Sun is well known for sensationalising everything.
In previous cases similar to this, the Sun has come out with long articles suggesting that the indications were there all the time and this or that wasn't done or ignored.
So far, no cohorboration of the claims.
This especially makes me suspect:
This is suggesting conspiracy, coverup, neglegence, incompetance, and complicity by the authorities.
Don't buy that.
Probably just the Sun trying to spin the story a bit until we hear otherwise.
