If the enquiry was into what the authorities could have done differently, it does seem that you should be looking at intelligence services and authorities, and hopefully they can learn from this and respond better next time.
If the enquiry was into who did it, we already knew that and it could have been finished in about 2 minutes and would have been pointless.

….and Al Beeb manage to avoid any mention of Salman Abedi the bomber …& Hashem Abedi who sourced explosive materials and planned the whole thing & the fact that they were Muslim extremists etc Instead , there weren’t enough stretchers available… Awful… Why would they mention the killers when talking about the aftermath and mistakes made that led to an increased death toll?

There seems to be a pattern emerging. Whenever ethnic types do a wrong thing, the blame gets shifted to "the authorities". Whether it's blowing up kids, rushing a venue without tickets, drowning themselves in the Med etc. This is just your race obsession talking. It’s an act of terror. Of COURSE they will question what further authorities could have done. They do that will all terror incidents.
